In the fifth cycle the teeth are checked for open contact areas between each tooth.
The doctor uses dental floss beginning on the upper right quadrant and proceeds to the upper left quadrant checking if floss can pass between each of the teeth.
An open contact is recorded by using two parallel lines between the diagram of the affected teeth.
The lower arch is also checked beginning on the lower left quadrant and proceeding to the lower right quadrant.

The sixth cycle involves using a full mouth set of periapical radiographs to supplement the clinical findings.
The doctor will examine the patient's data recordings on the examination form and compare the measurements against the radiographs beginning in the upper right molar quadrant and continuing around the upper teeth.
The doctor will then check the lower teeth beginning on the lower left quadrant and proceeding to the lower right quadrant.
The doctor will be checking to see if the pocket depths recorded are consistent with the radiographic bone level and to see if any bone loss is present in the furcation areas, or if any open contacts are visible.
Any new finding or changes will be recorded on the chart.
