For acute infections the drug of choice is penicillin VK or an extended spectrum penicillin:
Penicillin VK: Loading dose 1000mg followed by 500 mg 4 times daily for 7 days
Amoxicillin or Augmentin 250 mg 3 times daily for 10 days
If patient is penicillin allergic:
Erythromycin: Loading dose 1000mg followed by 500 mg 4 times daily for 7 days.
Chronic infection, inadequate response to penicillin:
Clindamycin: 300 mg 3 times daily for 7 days
