A method has been developed and frequently applied in cases of localized periodontal recession. Complete pre-op and post-op data on 10 patients , age 16-39. Facial probing depth of the involved teeth were between 0.5 mm and 2 mm avg.1.04 mm.
Surgical Method: A collar of tissue is excised from the receded gingiva in order to remove the sulcular epithelium. The root of the involved tooth is Sc/RP. Citric acid is applied. Partial thickness incision an envelope is created in the tissue around the denuded root surface. A wedge tissue is removed from the palate. The graft is placed in the envelope, so it covers the exposed root area.
Total root coverage was achieved in 5 of 12 cases. In the other cases coverage was from 60 to 83%. Result average was 80%. Gain in keratinized gingiva was avg. 3.5 mm, a gain in every case. Probing depth at these sites were between 1 and 2 mm.
Minimal trauma at surgical site. Good healing over deep wide areas of recession. Minor surface wound at donor site.
Cases used. Esthetics -Graft matches surrounding tissues., Localized areas with lack of keratinized and attached gingiva where inflammation cannot be controlled., Localized areas of recession with or without sufficient remaining keratinized and attached gingiva, Areas of recession next to crown margins, causing an esthetic problems.
