ANTIMICROBIALS: TOPICAL

Atsushi Saito et al: Locally Delivered Minocycline and Guided Tissue Regeneration to Treat Post-Juvenile Periodontitis. A Case Report. J Perio, 65:835-839, 1994.

Purpose:

This case report is a demonstration of a single case of post-juvenile periodontitis and a specific treatment regime worked on this one patient.

Materials and methods:

The patient is a 28 year old female of Asian descendant, who presents with angular defects in the incisors and first molar regions. Bacterial samples were taken.

Results:

Bacterial samplings show no Actinobacillus actinomycetemcomitans (Aa.) in the deeper lesions tested but did show Porphymonias gingivalis and Prevotella intermedia. Elevated serum immunoglobulin G of A. a., P. gingivalis and P. intermedia were observed. Topical delivery of minocycline, as part of the initial therapy for the purpose of suppressing the periodontopathic bacteria in the subgingival plaque, every week for 4 consecutive weeks which followed scaling and root planing and had significant clinical improvement by reducing gingival inflammation and suppuration and reducing the P. intermedia and P. gingivalis. GTR procedures were used and at two year follow-up showed no clinical breakdown. This is a limited study which tests a second generation tetracycline as an adjunct to GTR for post-juvenile periodontitis patients.
