ANTIMICROBIALS: TOPICAL

Purpose:
To study the clinical efficacy of 2 applications of Metronidazole 25% and 1 session of subgingival scaling in patients with adult periodontitis
Materials and Methods:
A total of 206 patients were included in this randomized, multi-center, split mouth design study. The patients with probing depth of > 5 mm at least in one of the 4 site were studied. Probing depth and bleeding on probing were recorded. Treatments were: 2 applications of dental gel and 1 week between applications and 2 session of subgingival scaling. Indices were recorded at 2, 6, 12, 18, and 24 weeks after treatment.
Results:
Probing depth in patients treated with the gel responded in the following way; 6% deteriorated 1 mm or more and 73% improved. In the patients that had scaling, 5 % deteriorated and 76% improved. Almost all patients had reduced their probing depths at the end of the follow up period. The reduction in the scaled pockets being somewhat greater than the gel. Both treatments were effective in reducing bleeding on probing and the effect persisted for at least 24 weeks after the end of the treatment. The reduction of bleeding on probing after scaling was significantly greater than that seen after the gel application. No serious adverse experiences were seen.
Conclusion:
The results of this study demonstrate that it is possible to achieve a clinical effect from local treatment of Metronidazole 25% lasting for at least 24 weeks and this result is comparable to subgingival scaling.
