Patient has not seen a dentist regularly. He had two episodes of abscess in the upper anterior teeth in the past few years and was recommended to see a periodontist. He is not aware of any oral habits and denies prior periodontal treatment. The patient did feel that his upper front teeth were changing position. He is intelligent and motivated to have all the treatment done.
