Patients last physical exam was in 1995. Patient does not recall any childhood diseases. Patient states that he has hypertention that was diagosed 5 years ago. He also has diabetes which started about 3 years ago. Patient currently utilizing Slipizide (20mg three times per day) as an oral medication to control diabetes. He is also taking oral Atenolol (50mg once per day) to control hypertension. Once every 6 months patient feels "short of breath". Patient also has sleep apnea. Patient does not smoke nor does he consume alcohol. Patient is mildly obsese. Lastly the patient has "Sciatica"- pain in lower back that radiated throughout his body. Patient received physical therapy and has not felt pain for 6 years. Patients family history: Bloodpressure on initial examination 127/74, pulse 72 bpm. Blood glucose level 190.
