The patient has recieved intermitant dental care ; 3 years ago tooth # 31 was extracted for periodontal reasons, and a night stabilization appliance for bruxism and tooth wear was created 3 years ago. Prior to that the patient was seen for occassional preventive and restorative purposes.
Current dental history
"Gum problems". Patient states that the problem started about 3 years ago and noted "sore gums" and bleeding. More acutely, the area around tooth #27 was painful at the time of initial examination and this had a duration of about three weeks in which time he noted pain and swelling. Patient also states that he is a bruxer. He has a night guard but does not wear it because he has sleep apnea and fears that this could interfere with his breathing, especially at night . The patient is not happy with his past dental care. He complains that the night guard did not fit properly and feels it was a waste of his time and money. His Oral hygiene routine consisted of brushing and flossing three times a day.
