  Systemic:            53 year old healthy patient
  
  Periodontium:        Generalized moderate chronic adult periodontitis
                       with localized severe bone loss.
  
  Dentition:           Partially edentualism.  Defective restorations.
                       Decreased masticatory efficiency.  Occlusal trauma.
  
  TMJ:                 No articular or muscular problems noted.
