  Patient arrived in the United States from Korea in 1978. Past dental treatment consisted of amalgam and composite restorations and PFM crowns. Maxilliary left second molar has been extracted and a fixed bridge placed from tooth #12-14. Periodontal recalls were in 6 to 12 month intervals. He brushes twice daily but he doesn't floss
