General Information
Class Location
We are located on the campus of the University of California Los Angeles, about 10 miles (16 km) North of Los Angeles International Airport. The majority of UCLA Continuing Dental Education courses are held on the UCLA Campus at the School of Dentistry. The School of Dentistry is located within the Center for Health Science (CHS) complex on Tiverton Avenue between Le Conte Avenue and Charles E. Young Drive South on the UCLA campus. Typically, course check-in takes place on the 1st floor lobby of the school.

Parking
Daily visitor parking can be obtained from attendants on Westwood Blvd. North of Charles E. Young Drive South for Lot #9, or at the Tiverton and LeConte entrance for the CHS Lot. A map and parking instructions will be sent to course participants who pre-register for courses. Parking costs for 'walk-up' course registrants is $10 per day (price effective August 1, 2008).

Payment can be made by cash (in person), personal or company check (from a U.S. Bank), Visa or MasterCard, or bank wire transfer. Amounts are payable in U.S. dollars. Registration applications are processed in the order they are received. Pre-registration is recommended.

Course Check-in
Typically, course check-in takes place on the 1st floor lobby of the school. Participants should bring their course enrollment confirmation letter and any items requested for use during the course.

Course Credits
UCLA CDE courses are issued continuing dental education credits on a credit per hour basis. For participants who complete our courses, receive credits approved by the California State Board of Dental Examiners. Participants are given a certificate of completion at the end of each course. We are also approved to issue Academy of General Dentistry (AGD) credits on a credit per hour basis. In additon, we are an ADA Continuing Education Recognition Program.

Enrollment Policy
Full attendance and course completion is required to obtain credit.

UCLA Affiliation: The UCLA School of Dentistry is not responsible for providing liability or malpractice insurance in workshop courses. Participants enrolled in courses which require patients will be asked to provide proof of such insurance coverage. No guests will be allowed in UCLA Continuing Dental Education courses

Refund Policy
Courses which do not receive sufficient enrollment are subject to cancellation. In the event a course cancelled or enrollment has reached capacity, UCLA will refund the full tuition paid.
In cases a participant needs to cancel, a full refund will be given for lecture courses if written notification of withdrawal is received 24 hours prior to the program. Refunds will be given for participation, hands-on, or special courses, less a cancellation fee (up to 40% of tuition), if written notification is received at least 10 working days prior to the program, unless otherwise specified. Notification of withdrawal received after a course’s beginning date will result in forfeiture of the entire course fee.
